Disability, Neurodiverse conditions, Mental health conditions and long-term health conditions

STEP 1:  Please download and complete the following form. You'll see that you only need to complete page 1 if you are not requesting adjustments or interviewer awareness, but if you have requests you must complete the whole form, get it signed by a member of staff at your school and provide supporting evidence.

STEP 2: If you are requesting adjustments, please get hold of the appropriate accompanying evidence document, as requested on the form. The document should do two things: 

1. confirm your disability, neurodiverse condition, mental health condition or long-term health condition.
2. (if relevant) confirm the adjustments / allowances you are currently being given or most recently have been given and the adjustments that you are expected to have in your final school exams, e.g. a letter confirming previous assessment, dates undertaken.

If you have a neurodiverse condition, please do NOT send a full Educational Psychologist report at this stage unless there is particular reference to the support you require that will help us make appropriate adjustments to our assessment and selection procedures. 

If your document is not in English, please provide the original and a translation (the translation can be done by you).

STEP 3: 
If you are only submitting the form (no adjustments or interviewer awareness needed), you just need to make ONE pdf file containing the form. If you are requesting adjustments or interviewer awareness, please make TWO pdf files, one containing the form, the other containing the evidence document. The files should not be larger than 3MB. If you need help with scanning, we recommend that you ask a friend or relative who has a scanner, ask your school, or use CamScanner, which is an app that allows you to scan on a smartphone.

STEP 4: Please save your pdf files with the file name following this example:

For the fictional Joe Bloggs applying for Natural Sciences (Physical) with UCAS Personal ID 1580174729, the file name would be

Please make sure 
> There are NO SPACES
> You use hypens 
> There is the number 5 at the beginning.
> If you are applying for Natural Sciences, please include (Physical) or (Biological) in all cases. 

STEP 5: Please upload your Adjustments form via the Adjustments form and document upload tool. You can only upload one thing at a time so you will need to upload the evidence document separately afterwards.

STEP 6: If relevant (if you have requested adjustments or interviewer awareness), please upload your Evidence document via the Adjustments form and document upload tool.

(POSSIBLE STEP 7): We will liaise with you if we have any questions about adjustments you have requested or if we need a further document relating to your disability, neurodiverse condition, mental health condition or long-term health condition)

STEP 8: Receive invitation for online interviews (if selected)
If you are selected for the online interviews, we will make the necessary arrangements for your adjustments (if required). Your invitation to interview will not normally make any reference to these, however. 

STEP 9: Receive adjustment confirmation email at the end of November / early December
We will email an adjustment confirmation to you shortly before your College admissions assessment (if relevant) or your interviews, confirming the arrangements that we have made for you (or confirming that we have not made adjustments if you do not require adjustments). Note that with the exception of adjustment emails for College admissions assessments, we can only do the work needed to send you this confirmation after we have sent invitations to interview in every subject (you can check our progress in Stage 4). We will update the box below once all adjustment confirmation emails have been sent.

We are aware that some applicants are reluctant to disclose a disability, neurodiverse condition, mental health condition or long-term health condition but we would like to reassure you that disclosure will in no way adversely affect the assessment of your application. 

The Adjustments form and document is what we look at when organising and preparing for interviews and assessments - it is used for practical information. In most cases, assuming that you have been given the necessary access arrangements for your learning and exams, all we will need is an adjustments form and document. 

If you have extenuating circumstances relating to your disability, neurodiverse condition, mental health condition or long-term health condition then your referee should mention these in the extenuating circumstances section of your UCAS reference. For example, if you were not given adjustments in your GCSE exams but should have had adjustments, this would be an extenuating circumstance which we could take into account when looking at your GCSE results. If more detail is needed than can be included on the UCAS reference, or if the information is particularly sensitive, then your referee may also complete an Extenuating Circumstances Form by 22 October. We do not need an Extenuating Circumstances information to set out what adjustments you get - the Adjustments form covers that and must be received in all cases. Extenuating Circumstances is about impact on your education or exam results.
